Physical Examination

• An updated Health History Update form must be completed for each season a student participates in a sport.
• Physical examinations are Valid for 13 months from the date of the actual Physical Examination. 

IMPORTANT:  Your doctor can provide the PHYSICAL EXAMINATION signed form.  Please remember that only physical examinations filled out and signed by a doctor will be accepted to participate in sports activities.

• You will be requested to upload the athlete's insurance card in Step # 5, please have the card ready to upload.
• If you would like to purchase School Insurance please download the Insurance Enrollment Form HERE.

HOW TO ENROLL

1. Decide whether you want the School Time, 24-hour Accident Protection (with or without Dental).
2. Fill out the enrollment form and enclose the form along with a check or money order made payable to AXIS Insurance Company for the correct amount.
3. Return the completed enrollment form and payment to your School. Your cancelled check or money order stub will be your receipt and confirmation of payment. (Please write the student's name and school name on your check.)
